A. In block letters, print your name, full address (including postal code) and telephone number (including area code) on a 8½" × 11" sheet of paper, and affix or staple three original UPCs (product bar codes) from any no fat no sugar added bread packaging (visit boulangeriestmethode.com for the list of eligible products) on this sheet. Only original UPCs of no fat no sugar added products will be accepted; all photocopies will be rejected.
B. Include a cheque or postal money order for $4.99 for shipping and handling, made out to “St-Méthode Lunch Bag”.
C. Mail the lot to: “St-Méthode Lunch Bag”, P.O. Box 295, 157 Place d’Armes, Montreal, Quebec H2Y 3J2, no later than March 4th, 2012. Requests postmarked later than this date will be null and void. Limit of one request per person, per address. Offer valid in Quebec only. Allow 6 to 8 weeks for delivery.
